I hereby agree to the following:
I acknowledge that I am participating in classes, workshops, webinars, or sessions offered by The Motion Space, LLC and Wendy Masterson. These classes, workshops, webinars and/or sessions may include somatic movement and movement re-patterning and facilitation, Reiki, the GYROTONIC®/GYROKINESIS® Method, dance, and fitness exercises during which I will receive information and instruction about somatic modalities, wellness techniques, movement and health. I recognize that any of the above activities may require physical exertion that may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved.
I represent and warrant that I am physically fit and I have no medical condition that would prevent my participation in these movement activities, health programs, or workshops. I understand that it is my responsibility to consult a physician prior to and regarding my participation in classes, workshops, or sessions offered by Wendy Masterson or The Motion Space, LLC.
In consideration of being permitted to participate in classes, sessions, training, webinars, or workshops, I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I might incur as a result of participating in the classes, sessions, training, or workshops.
I understand that classes, workshops, webinars, and sessions offered by Wendy Masterson or The Motion Space LLC require hands on contact, and I accept my responsibility to inform my trainer if any touch is uncomfortable.
I understand that it is my responsibility to self-regulate my behaviors and actions.
In further consideration of being permitted to participate in classes, sessions, training, webinars, or workshops, I knowingly, voluntarily and expressly waive any claim I may have against GYROTONIC®/GYROKINESIS® or Wendy Masterson or The Motion Space, LLC for injury or damages that I might sustain as a result of participating.
I, my heirs or legal representatives forever release, waive, discharge and covenant not to sue GYROTONIC®/GYROKINESIS® or Wendy Masterson or The Motion Space, LLC for any injury or death caused by their negligence or other acts.
I have read the above release and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above.
Video/Photo Release
I understand that The Motion Space LLC will be recording during the webinar. I hereby give and forever grant The Motion Space LLC and its licensees, successors, and assigns the right to use, publish, and copyright my picture and video portrait and likeness, in whole or in part, including alterations, modifications, derivations and composites thereof, on their website, in their printed publications in all media, no known or hereafter derived, that The Motion Space LLC deems appropriate.
In consideration of the printing/production deadlines, I hereby waive any right that I may have to inspect and approve the finished product or copy that may be used in connection with my portrait and likeness.
I also waive and release any claims that I may have against The Motion Space LLC and its licensees, successors, and assigns, based upon or arising from such uses, including without limitation , any moral rights of whatsoever description, any right to approve the photographs(s) or film(s), any claims for invasion of privacy, violation of the right of publicity, defamation, and copyright infringement or for any fees such use.
I have reached the legal age of majority in my state and am authorized to grant the permissions set forth therein.
